Probably it's a room prepared for the "Torture". Probably it was long time not used. It doesn't look used. The whole atmosphere in the picture is great. The light shines through the window, but isn't allowed to enter...
Textures and details are amazingly done. Especially the floor stunnes me. The wooden furniture is also perfectly done.
Technically one of the best images I've seen. It could be some artwork by "Blizzard" or some other company. Awesome.
The vision is - like said - very deep. Even the room looks old and hidden, one get's the feeling of the "user" is a very neat one. Also a professional one. Notes on the walls and little helpers on the board shos this to me.
I would have drawn more cruelity details, as torture-tools and so on. Despide of this I think it's great that there's no blood in the picture. That'd be too tawdry.
